    One Touch KC Collaboration Agreement


    This One Touch KC collaboration agreement documents the commitments of local agencies and organizations to support each other’s in providing resources and services to families and others in the Kansas City Metropolitan Area. The goal of this “One Touch KC” collaboration is to increase the number of people that receive services from each of the One Touch KC collaborators and enhance access to community resources and services in order to improve their health and well-being.



    Commitment

    One Touch KC collaborators will educate staff and modify program practices in order to utilize the One Touch KC intake forms to determine resource and service needs in the homes they visit. They will refer clients to other One Touch KC programs, where appropriate, to ensure clients receive a coordinated approach to address health, safety, housing, and energy efficiency needs.


    1. The One Touch KC collaborators will use consistent resource and service needs assessment tools and referral strategies to support this collaboration, as appropriate and feasible.
    2. One Touch KC collaborators will cooperate to improve integrated service delivery.
    3. One Touch KC collaborators will work together to collect and share data as part of this collaboration.
    4. The participating organizations, agencies and collaborating programs will take best efforts to keep all information received from participants confidential and shall not use or disclose this information for any other purpose without prior written consent of the client.
    5. No provision of this Collaboration Agreement may be waived or modified except by agreement in writing signed by the waiving or modifying agency or organization. A waiver of any terms or provision shall not be construed as a waiver or modification of any other term or provision. The parties agree to take action to amend this Collaboration Agreement from time to time as necessary.
    6. The persons signing below have the right and authority to execute this Collaboration Agreement and no further approvals are necessary to create an agreement.

    Effective Date


    The provisions of this Collaboration Agreement shall be effective as of the date of the last signature and shall terminate without cause at any time by providing not less than thirty days prior written notice.

    IN WITNESS THEREOF, the parties have executed this agreement, effective upon the date set for below.
